I give permission for my child’s picture, moving pictures, or any other graphic depiction or likeness, to be used by the Boys & Girls Clubs of Elkhart County, Inc. and for its activities. I give permission to the Boys & Girls Clubs of Elkhart County, Inc. to seek emergency medical treatment for my minor child if I cannot be reached. I will be responsible for any/all costs of medical attention and treatment. I understand KidsCare is a controlled access program and I am responsible for my child’s transportation to and from the appropriate KidsCare site. I understand that payment is due on the day of service.
SMS Terms and Conditions
Boys & Girls Clubs of Elkhart County (BGCEC) is committed to building user trust and confidence by promoting and complying with the use of business practices that help protect the privacy of the user and his/her data.
BGCEC offers recurring text message services. By opting-in to any such service, you are confirming your entry into a recurring texting program and you give your consent to BGCEC to use an automatic system to deliver text messages to the phone number to which you have directed it to transmit text messages. Message frequency varies.
BGCEC does not charge for our texting service; however, message and data rates may apply from your mobile carrier. Subject to the terms and conditions of your mobile carrier, you may receive text messages sent to your mobile phone. By providing your consent to participate in our texting service, you approve any such charges from your mobile carrier. Charges for text messages may appear on your mobile phone bill or be deducted from your prepaid balance.
BGCEC reserves the right to terminate the SMS service, in whole or in part, at any time without notice. The information in any message may be subject to certain time lags and/or delays.
To stop receiving text messages, text “STOP” or “QUIT”. By enrolling in the texting service, you consent that after having submitted a request to opt-out you will receive one (1) final message confirming that you have been removed from the system. Following the opt-out confirmation message, no additional text messages will be sent unless you rejoin the system from your mobile device.

Distance-Based Programming/Club at Home
Dear Parent/Guardian:
In an effort to continuously serve members during the Club closure, Boys & Girls Clubs of Elkhart County is providing distance-based Club experiences through which Club staff will facilitate program activities through an online platform. Boys & Girls Clubs of Elkhart County will use applications provided by a third party that members, parents/guardians and/or staff will access via the Internet and use for purposes of communication and programming. The primary platforms are called Zoom and Stride Academy.
This letter seeks consent for your child to utilize Zoom for distance-based Club program purposes. Zoom provides an opportunity to deliver video and chat-based educational experiences to members via any device. Please be aware that Zoom collects information about its users and has its own privacy terms and conditions to which members must adhere. Please review Zoom’s privacy terms and conditions carefully before registering your member: http://zoom.us/terms and http://zoom.us/privacy. You agree to hold harmless Boys & Girls Clubs Elkhart County while utilizing these platforms.
In order to participate in distance-based Club experiences, you will need to provide the following:

A computer, mobile, or tablet device with access to the Internet

A quiet space at home in which members can participate in distance-based experiences under the supervision of an adult

To register for Zoom, you will need to provide some customer data, including but not limited to: your email address and first and last name (for more information, see https://zoom.us/privacy)
We will use Zoom for the following anticipated program activities:

Media sharing (for example, uploading images of artwork or other projects)
Stride Academy is an adaptive learning program that personalizes learning for every student, based on their needs. Stride Academy gathers data about student performance in order to diagnose skill gaps; to provide a variety of online skills practice, video lessons and printable lessons; to monitor progress. While limited, Club may also utilize social media to promote events and communicate one way with your youth. Parents/Guardians will determine the appropriate social media for their youth as social media (e.g. Facebook) regularly collect information from its users.
Our commitment to keeping the young people we serve safe is always our number one priority. Boys & Girls Clubs of Elkhart County will actively monitor member activity on Zoom and will make every effort to protect member information by, among other things, maintaining control of, and access to, the data collected; prohibiting re-disclosure of member information; limiting the purposes for which the online platforms may use member information; ensuring there is no advertising and that no member information is collected for commercial purposes.
Further, all activities online must comply with Boys & Girls Clubs of Elkhart County’s safety policies, including the Code of Conduct, Prohibition of 1:1 Contact Policy, which are available in our Parent Handbook.
